The social prescribers work with patients to help them to improve their health and wellbeing, have better social connections, reduce stress and anxiety, improve confidence, have more control over their lives and better manage long term conditions. A social prescriber works with you to identify what matters to you and to create a joint action plan to help you achieve your goals.
Social prescribers can give you the time and confidence to work on underlying issues that affect your health and wellbeing. They can also connect you to organisations and sources of support in the community and refer you to other organisations where appropriate.
