Use this service to request an ADHD assessment.

Your GP has asked you to submit this form because you asked for an ADHD assessment. This information explains what ADHD is, who may qualify for an NHS-funded assessment, and what to expect during the process. Please take time to read and understand this information before you decide how to proceed. If you need help completing this form, please contact your GP practice to ask about support from a social prescriber.

ADHD (Attention Deficit Hyperactivity Disorder) is a lifelong neurodevelopmental condition that affects attention, impulse control, and activity levels. Symptoms begin in childhood and are usually present across different areas of life, such as home, education, work, and relationships.

National clinical guidance advises that patients should only be referred for an ADHD assessment if symptoms started in childhood, cannot be explained by another psychiatric condition, and are associated with moderate or severe psychological, social, educational, or occupational impairment. Although ADHD is relatively common, not everyone who experiences these symptoms requires a diagnosis.

The questions in this form are designed to understand how ADHD-related traits may be affecting your daily life and whether referral for an ADHD assessment is appropriate. A section of the form must be completed by someone who knew you as a child and can comment on both past and current challenges.

The information you provide, together with information from your GP and your associate, will be shared with NHS Midlands and Lancashire Commissioning Support Unit. This information will be used to determine whether you meet the criteria for an NHS-funded ADHD assessment and whether a Right to Choose provider is able to accept a referral. Your GP will be notified of the outcome within 20 working days (4 weeks), and a copy of the outcome letter will be sent to the email address you provide.

If you are approved for assessment, your information will be shared with the provider you nominate, unless it is decided that they cannot meet your clinical needs, in which case you will be referred to a locally commissioned mental health provider. If you are not eligible for an NHS-funded assessment, you will be provided with a list of useful resources.

ADHD assessments typically take around 2 hours, and further appointments may be required. By submitting this form, you agree to attend all arranged appointments. If appointments are missed, you may be removed from the assessment and treatment pathway.

If diagnosed with ADHD by a Right to Choose provider, the treatment they can offer is medication only. If you would prefer non-pharmaceutical treatment options, please ask your GP about referral to local mental health services, which may offer alternative support.
